New Clients/Patients
If you are new to our practice, please review our new client and patient registration forms prior to your arrival.
We ask that all clients attempt to arrive approximately 15 minutes early for all appointments. If you are transferring from another veterinarian's office or have been referred to OPVMC by your primary care doctor, you should also bring with you a copy of your pet's records, any x-rays, or test results. All referrals should be accompanied by a referral letter from your primary veterinarian.
